Dear Travel Agent,
We inform you that, starting November 3, 2026, all passengers traveling to the United States of America or transiting through U.S. territory will be required to present a mandatory attestation form before boarding.
This requirement is intended to confirm that the passenger has not been present in the Democratic Republic of the Congo (DRC) during the 21 days preceding travel, including transit through airports in that country.
What does the passenger need to do?
- Download the attestation form using the following link:
Ebola Attestation Form - Complete, sign, and date the attestation with the date of travel before departure.
- Submit the document at check-in, together with all other required travel documentation.
Important Information
- The attestation is mandatory for boarding.
- Passengers who do not present a properly completed, signed, and dated attestation bearing their flight date may be denied boarding.
- We recommend that passengers arrive at the airport with additional time to allow for verification of all required documentation.
